Optional Health Insurance Benefits

In addition to the medical plan, you or your employees have the option to join the following programs

  • Health Care Flexible Spending Account (FSA) – Allows you to contribute up to an annual maximum of $2,500 if you’re are a full-time employee, or up to $1,250 if you’re a part-time employee. This plan provides tax-effective reimbursement of eligible health care expenses such as deductibles, co-pays, dental, vision care, eyeglasses, hearing aids and other items not covered by the medical plan.
  • Dependent Care FSA – You may contribute up to $5,000 per year (up to $2,500 per year if you’re married and file separate returns) on a pre-tax basis for reimbursement of eligible dependent day care expenses
  • 401(k) Savings Plan – The plan has the following features:
  • You may make pre-tax savings of up to $16,500 in 2010 (subject to adjustment for future years).
  • If you’ll be age 50 or older on December 31, 2010, you may make an additional pre-tax catch-up contribution of up to $5,500 (subject to adjustment for future years).
  • Short-Term Disability Insurance – Starting after the seventh day of illness, the plan pays 60% of your base pay, up to $500 a week, for up to 26 weeks.
  • Long-Term disability Insurance – If you qualify after six months of disability, you'll receive monthly disability payments equal to 60% of your pay (up to $10,000 per month), offset by other disability benefits you’re eligible to receive. These payments will continue for as long as you qualify as disabled, up to age 65 or for a specified period if you are disabled at age 60 or later.
  • Life & ADD&D Insurance – The plan provides coverage equal to two times your base pay (rounded to the next highest $1,000) up to $150,000. For coverage above $150,000, you'll need to provide satisfactory proof of good health.

 

In addition to your contributions for medical coverage, your contributions to the flexible spending accounts, and for short- and long term disability, and life insurance of less than $50,000, are made with pre-tax dollars.
